I want to add my spouse's name to my online profile.

Adding your spouse's name to your online profile is an easy thing to do, but before that, you need to read the instructions below:

1. Did you change your legal name after getting married? If yes, please read this article HERE
2. If you did not have to change your last name after getting married, please ask your spouse to fill the forms below:
   
   • Your spouse needs to sign Mission Quest's Statement of Faith. (Attached to this article)
   • Your spouse needs to sign the Agreement of Integrity. (Attached to this article)
   • Your spouse needs to sign Mission Quest's Release of Liability. (Attached to this article)
